Paranoia

I think Macbeth's paranoia is due to the consequences of what he did.  His paranoia is acting up more once Banquo has been killed and right after he said something like "To our friend Banquo." That's when everything started to reveal its self in front of Macbeth. He is the only one so far with paranoia and its going to continue until Macbeth admits to what he has done is wrong. And confesses to the murders of King Duncan and his beloved friend Banquo.
